What is a Patient-Centered Medical Home?
NCQA’s Patient-Centered Specialty Practice Recognition program recognizes specialists that deliver high-quality, patient-centered care. The program focuses on sharing information with the primary care clinician and working as a team to coordinate care with other referring clinicians, community resources and secondary services. Care is focused on the patient, rather than on the care setting.
Who is part of your Patient-Centered Medical Home team?
Your health care provider team.

Most importantly- YOU! You are the most important person on your health care team. Patient-centered is a way of saying that you are the focus of your health care.
What do you need to do as part of your Patient-Centered Medical Home team?
Keep your medical home providers informed! Make sure we have accurate contact information for you and your emergency contacts.
Please call the office prior to making any changes to your health insurance plan to confirm we accept that plan.
Let your health care provider know about care you receive from other health care professionals.
Before you go to an Urgent Care or Emergency Room — Call our office first –We offer same-day appointments.

After Hours, please call our office number and our dedicated answering service will be in touch with our medical providers for you.
Let your medical home provider know if you have been in the hospital.
Bring all your medications (or a list of all your medications) with you to each office visit.
Take an active role in your own health. Practice healthy lifestyle choices.
Follow the health care plan that you and your team agreed on.
Set goals that you can reach. Once these goals have been reached discuss new goals.
Have all your preventative screenings done—i.e. eye exam, mammogram, colorectal screening, annual bloodwork, and annual physical exam- just to name a few.

Help you manage your health care – including any chronic conditions.
Help answer all your health questions and Listen to your concerns.
Coordinate your care if additional services are needed.
